Instructions for Completing the FMP Forms
When filling out the Foreign Medical Program (FMP) forms, please keep the following details in mind:

1. Physical Address:

• The physical address must be in the Dominican Republic, as this is where you will receive treatment.
• If you do not yet have a Dominican address, you can use the hotel address where you will be staying:

-Boutique Hotel Las Flores

– Calle Las Flores

– 23000 Punta Cana

2. Mailing Address:
– Use the address that is registered with the VA for your mailing address.

3. Signature and Date:

• Please provide a digital signature. If you are unsure how to sign digitally, click here to learn how.
• Ensure the form is dated appropriately.
